ABSTRACT

Sporotrichosis associated with exposure to domestic cats is hyperendemic in Rio de Janeiro, Brazil. A review of the clinical records at our institute revealed four patients with clinical signs of dacryocystitis and a positive conjunctival culture for Sporothrix who were diagnosed with Sporothrix dacryocystitis. Three patients were children (< 13 years of age) and one patient was an adult. Two patients reported contact with a cat that had sporotrichosis. Dacryocystitis was associated with nodular, ulcerated lesions on the face of one patient and with granulomatous conjunctivitis in two patients; however, this condition manifested as an isolated disease in another patient. All of the patients were cured of the fungal infections, but three patients had chronic dacryocystitis and one patient developed a cutaneous fistula. Sporotrichosis is usually a benign disease, but may cause severe complications when the eye and the adnexa are affected. Physicians, especially ophthalmologists in endemic areas, should be aware of the ophthalmological manifestations and complications of sporotrichosis.

ABSTRACT

Foram examinados pelo autor 565 pacientes HIV+ no período de 1996 a dezembro de 1998. Os pacientes que apresentaram alterações oftalmológicas secundárias a infe~ção pelo Cryptococcus neoformans foram selecionados. Dos 18 pacietnes selecionados, 12 apresentavam edema do disco óptico, 3 atrofia óptica, 3 paralesia de IV nervos e 2 coroidite multifocal. O autor discute sobre a fisiopatologia das alterações bem como o prognóstico visual.

ABSTRACT

A neuropatia óptica isquêmica anterior (NOIA) tem como causas principais a arteriosclerose (forma näo arterítica) e a arterite temporal (forma arterítica). Fatores de risco como a hipertensäo arterial, diabete e doença coronariana säo importantes na gênese do primeiro grupo. Outras causas pouco frequêntes tais como doença oclusiva da carótida, fibrilaçäo atrial, cirurgia da catarata, colagenoses e raramente fenômenos ambólicos säo citadas. O objetivo do trabalho é a apresentaçäo de um caso de neuropatia óptica isquêmica anterior secundária a embolismo ocorrido durante cateterismo cardíaco, fato apenas relatado em um caso de literatura.
